weight loss in the elderly in long term care settings

Unintentional or involuntary weight loss is a common phenomenon among the elderly. Knowing the risk factors, differential diagnosis, prognosis, and available treatment can prevent someone from having adverse reactions from weight loss. Every long term care facility has a policy in place, and every individual is unique so  certain interventions should be modified to meet the individual needs. This paper should address that. My instructor uses two plagiarism checks on papers so please be aware that this has to have some originality
